Interim Report Instructions


*Only complete this report form if you were awarded a grant in the MOUNT VERNON ARTS INITIATIVE grant category. If you were awarded a Basic Program Support, Project Support, Arts Alive or Yonkers Arts Initiative grant please return to the main page and click on the form that corresponds to your awarded grant category.

Submit this report only if you have not completed your funded project by February 2019.  If you complete your project between July 2018 and February 2019 please submit a FINAL report only.
The interim report is due on or before Tuesday, February 12, 2019. Interim Reports must be submitted before we can release any remaining payments. 


Contract Period: July 1, 2018 through June 30, 2019.
