Interim Report Instructions

*Only complete this report form if you were awarded a grant in the BASIC PROGRAM SUPPORT grant category. If you were awarded a Project Support, Arts Alive, Mount Vernon Arts Initiative or Yonkers Arts Initiative grant please return to the main page and click on the form that corresponds to your awarded grant category. 

Please complete an Interim Report for your organization's 2018-19 Basic Program Support grant. The interim report is due by or before Friday, February 22, 2019. Interim Reports must be submitted before we can release any remaining payments. 

Contract Period: July 1, 2018 through June 30, 2019. 

This Interim Reporting Period:  July 1, 2018 through December 30, 2018
