Interim Report Instructions

*Only complete this report form if you were awarded a grant in the PROJECT SUPPORT grant category. If you were awarded a Basic Program Support, Arts Alive, Mount Vernon Arts Initiative or Yonkers Arts Initiative grant please return to the main page and click on the form that corresponds to your awarded grant category. 


Please complete an Interim Report for your organization's 2018-19 Project Support project. Interim reports are due on or before Tuesday, February 12, 2019. Interim Reports must be submitted before we can release any remaining payments. 

Contract Period: July 1, 2018 through June 30, 2019. 

This Interim Reporting Period:  July 1, 2018 through February 12, 2019


If you have completed your funded project, please submit a Final Report only.


Please note: You must submit a separate Interim Report for each project you were awarded. You will only be able to work on one Project Support interim report at a time.
